Accepted payments include cash, check, all major credit cards, Health Savings Plans (HSA), and Flexible Spending Accounts (FSA). Payment is due at the time services are rendered. Results will not be delivered until payment has been received.
*A 10% deposit is due for evaluation at the time the appointment is scheduled.
*credit/debit charges will be charged a fee of 2.75%.
* Returned checks will result in a $50 fee.

 

There are no hidden fees or extra charges for autism evaluation. fees are based not only on the services but the level of education, experience, and expertise that comes with the provider. Please contact me for my fee structure.

 

Dr. Lake is not paneled with insurances as they may restrict the nature of the evaluations/tests that can be completed. Dr. Lake encourages you to contact your insurance company in advance if you are interested in getting reimbursement. Testing codes are 96130 96131, 96136, 96137. Dr. Lake can provide the appropriate billing and diagnostics codes (if appropriate) following the completion of the evaluation.
